To apply, the candidate must submit a current curriculum vitae, three letters of recommendation (ideally from the program attended and/or clinical preceptors), and a letter of intent. 

The letter of intent should describe the applicant’s motivation and interest in the fellowship and how they feel their involvement will benefit the program University of Utah Hospital or Huntsman Cancer Hospital. They will then need to email the directors of the track they are interested in (see contacts below). Emails should also include their letter of intent and current curriculum vitae. Additionally, applicants should ask three professional references (ideally from the program attended and/or clinical preceptors) to email a letter of reference directly to the directors. Letters of recommendation sent through the applicants will not be accepted.

The applicants will also be required to provide evidence of successful completion of an accredited Acute Care Nurse Practitioner program or accredited Physician Assistant program, as well as proof of one’s board certification and licensure to practice and prescribe medications (I.e. an active DEA license).

Following review of the aforementioned documents, those selected will be invited for an interview, which may be conducted virtually or in person depending upon the current public health recommendations and applicant availability. This interview will consist of two parts, first, the applicant will be asked to further discuss their interests and qualifications for participating in the fellowship. The second portion of the interview will consist of a short case scenario discussion. Following this interview, the applicant may be asked to participate in a second interview with the hospital leadership before a final decision is made. 

Once accepted to the fellowship, the new fellow must fulfill all University of Utah HR requirements, which includes completion of a formal online application through the employment website, HIPAA training, and proof of immunizations and active ACLS/BLS certifications. The applicant must also submit their NPI information to HR prior to requesting privileges to practice.
